    As Charles wrote, most likely what you are seeing has been caused by the presence of direct bold formatting in the headings in your document.     An even simpler approach is to use Ctrl-F9 and type TOC between the field braces, then press F9 to update. As for the entry layouts, those are controlled by the TOC 1 through TOC 9 Styles.
